ATTENDING THE PRACTICE
patient, the routine for attending the practice will have changed since
your last visit. Please read the following carefully.

the toilet beforehand. Please bring a pen to sign your
paperwork and be prepared to pay before you appointment.

to wait in your car or in our designated waiting area. On arrival
please telephone reception. When we are ready to see you we will ask you
make your way to the front door where you will be met by a nurse wearing
PPE. Please wear a mask. Your temperature will be taken and you will be asked to use hand
sanitiser. You will need to leave your
coat and belongings in a designated area, you will then be escorted to
the surgery for your treatment. After your treatment you will be
escorted by the nurse to reception.

Should you answer yes to any of the questions we will still be able to offer you treatment. It is important that you call us and do not attend the practice.
also need to know if you are shielding (staying at home as you are extremely vulnerable to COVID-19). Patients who are
shielding will have received a letter from NHS England.
Please let us know if you are vulnerable but not shielding. Anyone over 70 years old is considered to be vulnerable.
